The forms listed below are the forms that we will give you when you visit our office. To save yourself time, print them out and and have them ready when you get here.Patient Information Forms Appointment Hours: Monday-Friday: 8:30 a.m.-11:30 a.m.; 1:30 p.m.-4:30 p.m. Friday: 9:15 a.m.-11:30 a.m. We are located in Wildewood Business Park. Turn between the Burger King and Sonic on Two Notch Road next to the Wal-Mart. Our building will be on the left. John L. Mason, OD Appts: Tuesdays & Wednesdays All Day; Monday Mornings Only. Will see patients of all ages. Katie S. Mason, OD Appts: Monday & Thursday All Day; Wednesday & Friday Mornings Only. Will see patients of all ages. Dr. Katie normally examines our pediatric patients. - Insurances we take for routine vision exams: Medicaid, Medicare*, BlueCross State Health Plan ($60 exam fee), VSP, Davis Vision, EyeMed, Cigna Vision. If you were not given a card for your vision insurance, you may may need to contact your medical insurance company in order to determine your vision coverage for routine eye examinations. If you are self-pay (do not have vision insurance), the cost of a routine vision exam is $80. The contact lens fitting fee is $40 to $60 depending upon the type of lens you wear and difficulty of fit. - Insurances we take for medical visits (follow ups after a routine vision exam, eye infections, foreign body removal, corneal abrasions, etc): BlueCross State, BlueCross BlueShield, Blue Choice*, Medicaid, Medicare*, Cigna, Aetna *Medicare may have a $26 dollar charge for the refraction unless a secondary insurance allows the charge & we do not accept Railroad Medicare. *BlueChoice - If your plan is not a PPO, a referral from the primary care physician is required prior to services being rendered in our office. -Payment Options - We do not accept checks. We accept Cash, Visa, MasterCard & Discover. |
